ADHD Paperwork

Please click on the link below and print the form (4 pages).  Please fill out the "Parent" form and have one of your child's teachers fill out the "Teacher" form.  When they are complete send them back to the office at info@dixiepediatrics.com or drop them off in person 2 days prior to your scheduled visit so we have time to look them over and score them.
